Registration Information

The following forms are available for the 2009/2010 season. 

1. Club Registration Form - Boys

    $50.00 fee due when submitted to the SCVA office

2. Club Registration Form - Girls

    $50.00 fee due when submitted to the SCVA  office

3. SCVA Facility Insurance Request Form

    $50.00 fee when applying for liability insurance for practice facilities.

4. Adult Registration Form

    $45.00 registration fee due when submitted to the SCVA office.

    All adults registering as a Club Director, Adminstrator, Team Representative,

    Coach, or Chaperone will need to pass a background check prior to the

5. Background Consent and Waiver Form

   $18.00 processing fee is due when submitted to the SCVA office.

    All adults registering as a Club Director, Adminstrator, Team Representative,

    Coach, or Chaperone will need to pass a background check prior to the

6. Club Personnel Code of Ethics Form

    All administrators, directors, coaches, and other club personnel must complete

    the Code of Ethics Form and it must be on file with the SCVA in order for his/her

    registration to be considered complete.

7. Chaperone Responsibility Form

    All chaperones must submit this form with the Adult Registration form and               background screening form.

8. Junior Registration Form

    $45.00 registration fee due when submitted to the SCVA office.  Normally

    registration forms are included with the team registration form or the individual

    add/transfer form.

9. Medical Release Form

    Usually submitted to the SCVA with the individual's registration form.  Medical

    Release form is required for all inter-regional events.

10. Team Registration Form

    $15.00 registration fee is due when submitted to the SCVA office.

11. Individual Add/Transfer Form

    Individual registration forms must be submitted with the add/transfer form.
