Registration Information

The following forms are available for the 2007/2008 season. 

1. Club Registration Form - Boys

    $50.00 fee due when submitted to the SCVA office

2. Club Registration Form - Girls

    $50.00 fee due when submitted to the SCVA  office

3. SCVA Facility Insurance Request

    $50.00 fee when applying for liability insurance for practice facilities.

4. Adult Registration Form (New for 2007/2008)

    $45.00 registration fee due when submitted to the SCVA office.

    All adults registering as a Club Director, Adminstrator, Team Representative,

    Coach, or Chaperone will need to pass a background check prior to the

5. Background Screening Consent and Waiver Form

   $16.00 processing fee is due when submitted to the SCVA office.

    All adults registering as a Club Director, Adminstrator, Team Representative,

    Coach, or Chaperone will need to pass a background check prior to the

6. Chaperone Responsibility Form (New for 2007/2008)

    All chaperones must submit this form with the Adult Registration form and               background screening form.

7. Junior Registration Form (New for 2007/2008)

    $45.00 registration fee due when submitted to the SCVA office.  Normally

    registration forms are included with the team registration form or the individual

    add/transfer form.

8. Medical Release Form (New for 2007/2008)

    Usually submitted to the SCVA with the individual's registration form.  Medical

    Release form is required for all inter-regional events.

9. Team Registration Form

    $15.00 registration fee is due when submitted to the SCVA office.

10. Individual Add/Transfer Form

    Individual registration forms must be submitted with the add/transfer form.

11. Coaches Code of Ethics

    Usually submitted with registration form.
